Local Dynamic Micromassage–of Wellcomet® is an innovative technology which was specially developed to treat different aesthetical and dermatological conditions.

Local Dynamic Micromassage is a new type of treatment that is qualitatively different from other types of massages. LDM®-Technology utilizes specific ultrasonic waves whose frequencies can oscillate with great speed. This generates effects not achievable with conventional ultrasound.

Ultrasound therapy in aesthetic medicine and dermatology

Among the most important parameters of an ultrasonic wave are its intensity (sonic energy being introduced into the body per second and square centimeter; unit of measure W/cm2) and frequency, which determines, among other things, how deep the ultrasound waves will penetrate into the body. Medical applications (primarily related to physical therapy) use ultrasonic waves with frequencies of 1 MHz and 3 MHz, which penetrate relatively deep into the body. However, in dermatological or aesthetic applications the most important target for treatment is the superficial epidermis and dermis. Using ultrasonic waves with significantly higher frequencies is particularly advantageous in this case.

For example, the half-value depth (the distance at which half the initial energy has been absorbed) for waves with a frequency of 1 MHz is approx. 3 cm, for 2MHz approx. 1cm. For ultrasonic waves with a frequency of 10 MHz this value is approx. 0.3 cm and thus, when used for dermatological and aesthetic applications, this higher frequency is clearly superior to other frequencies and aesthetic applications, this higher frequency is clearly superior to other frequencies

The wave of this frequency has additionally a very small wavelength of 150 µm and can so directly influence single cells or small cell groups within tissue. This property is responsible for some very specific reactions of different cells to ultrasound waves of 10 MHz frequency. Ultrasound with its diverse mechanical, thermal and chemical effects on cells and tissue structures has already been in use for a long time in several areas of medicine and in aesthetic applications. Ultrasound energy may be converted into thermal energy by the absorption of ultrasonic waves in the tissue. The thermal energy distribution is strongly dependent on the intensity and frequency of the ultrasound. Thermal energy can lead to the stimulation of blood circulation and lymphatic transport as well as to metabolic activation. Ultrasonic waves exert a strong mechanical effect on biological structures. This effect is primarily due to rapid changes in pressure within the tissue. This process may accelerate tissue diffusion processes, stimulate metabolism and increase membrane permeability, among other things. This type of micro- massage can activate fibroblasts, which leads to a significant improvement in skin structure, increased tissue elasticity and turgor as well as to connective tissue tightening. Ultrasonic energy may also cause chemical changes in the tissue. Among these, the most important are: changes in collagen cross-linking, gel-sol transitions in the extracellular matrix and pH changes of the skin.

LDM®-Technology

LDM® – Local Dynamic Micromassage – is a new treatment method that is qualitatively different from other types of massages. LDM® Technology is based on rapid ultrasonic frequency oscillations (100 to 1000 times per second). As a result, pulse pressure gradients will oscillate in the tissue with the same frequency. This leads to a new type of tissue micro-massage that is not achievable with conventional ultrasound applications. If the ultrasonic frequency will be not kept constant, but instead steadily oscillates between two frequencies, the absolute pressure in tissue at a given sound intensity will remain constant, but the dynamic pressure gradients will oscillate with the frequency. This provides a unique opportunity to modulate and enhance the ultrasound massage effects. The new LDM® Technology is bas ed on this type of modulation. Fig. 2 shows the composition of a LDM® wave.

The LDM®-MED device features an enhancement of this technology – the LDM® -Technology. With this technology, the duration of each frequency transmission (pulse train) can be set separately (Fig. 3). This capability allows a more precise and subtle setting of the LDM® effect.

LDM®-Technology is based on the most current scientific findings in connective tissue metabolism. These findings increasingly emphasize the importance of degradation processes rather than generation processes in dynamic connective tissue equilibrium. In the meantime, the long established Collagenases have been joined by an entire family of matrix metalloproteinases (MMPs), which now comprises 29 members.
This group of enzymes appears to be involved in connective tissue metabolic disorders that manifest themselves in various medical conditions (e.g. acne, eczemas, dermatitis, chronic leg ulcer and even cancer) and may also be involved in skin aging (skin aging related to UV exposure in particular). In skin affected by acne, for example, the MMP-1 values can be 500 to 1000 times above normal levels.
Thus, any therapeutic procedure that can exert an inhibitory effect on MMPs (MMP-2 in particular) may reduce the damaging effect of these substances on tissue or may even completely arrest this process for a finite period of time. Collagen metabolism may be returned back to normal by temporary MMP reduction. LDM®Technology can significantly modify MMP activities in tissue and thus a therapeutic effect can be achieved for all dermatological and aesthetic indications that are path physiologically linked to abnormal MMP activities.

What is the treatment schedule of LDM®-MED?

Improvement of the skin appearance can be often seen directly after the first session. Dependent on customer age and skin condition aesthetical treatments are recommended twice per week for 4-6 weeks while maintenance sessions should be done once every 2-3 months. Dermatological applications may need 6-8 and in complex cases 10-16 sessions, depending on disease picture. As support for aesthetic-plastic surgery LDM®-MED can normally be applied 1-2 times before and 2-3 times after surgery.
